PARENTAL PERMISSION WAIVER

I hereby release and discharge any and all claims against either The Nick Bell Memorial Mentoring Foundation or Fultondale High School, for damages, personal injury and property damage in which my child or any other person may incur as a result of participation in this camp. I hereby authorize the agents & volunteers of the Camp to act in my behalf using their best judgment in any emergency, including medical. All photos/representations taken during the camp are owned solely by the Nick Bell Foundation. I have read this waiver, fully understand its contents, and request to be included in the Nick Bell Mentoring Memorial Foundation Football , Basketball, Cheer, and Baseball Camp.
